Job summary

MARIONVALLEY is seeking an RN to oversee the resident assessment process, MDS coordination, and care planning in a skilled nursing facility. The role emphasizes accurate data entry, regulatory compliance, and leadership in MDS processes.

You will supervise, train, and coordinate multidisciplinary teams, ensuring timely completion of MDS assessments and adherence to federal and state guidelines. This position requires strong organizational and analytical skills.

Qualifications

• Graduation from an Accredited School of Nursing with BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficiency with Microsoft Office; PCC knowledge preferred.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines.
• Lead weekly PDPM meetings for skilled patients and track tool utilization.
• Monitor QM and QASP performance with monthly/quarterly data analysis and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Coordinate and document completion of MDS assessments and care plans for new admissions.
• Maintain confidentiality of resident care information and manage data entry for MDS submissions.
