Job summary

Qualifications

• BSN preferred and current state licensure in good standing.
• 3 years supervisory experience preferred.
• Long Term Care experience preferred.
• Knowledge of PCC and regulatory requirements is desirable.
• Must maintain required continuing education/licensing.

Responsibilities

• Oversee weekly PDPM meetings for skilled patients.
• Track PDPM tools and data accuracy for documentation and reporting.
• Analyze QM and QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new coordinators.
• Assess health status and level of care for all new admissions.
• Ensure timely completion of MDS assessments and PPS Medicare processes.
• Coordinate interdisciplinary MDS involvement within regulatory time frames.
• Maintain accurate MDS scheduling and electronic submissions.
