Job summary

Pacific Villas Post Acute seeks an experienced Nursing Supervisor to oversee the facility's resident assessment processes, including MDS, PPS and care planning, ensuring guidelines are met and data quality is maintained.

You will chair weekly PDPM meetings, monitor MDS timing and accuracy, train new coordinators, ensure regulatory compliance, and coordinate interdisciplinary teams to deliver safe, effective, and compassionate care for all residents.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain all required continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Must maintain all required continuing education/licensing.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines.
• Lead weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools utilization.
• Monitor QM and QASP performance; provide data-driven recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Coordinate MDS submissions and ensure regulatory timeframes.
• Maintain confidentiality of resident care information.
• Schedule and conduct resident care conferences and care plan reviews.
• Ensure accurate data entry and electronic submission of MDS.
• Communicate level of care with multidisciplinary teams.
