Job summary

Antelope Valley Care Center is seeking a Nurse Manager to oversee the facility and ensure assessments are completed per guidelines, with a focus on the resident assessment process, MDS assessments, and the resident plan of care.

You will supervise staff, participate in PDPM and MDS training, and coordinate interdisciplinary care conferences to ensure timely and compliant documentation and care planning for new admissions and ongoing residents.

Qualifications

• BSN or equivalent nursing degree from an accredited program.
• Must be in good standing with the State Board of Nursing; maintain licensure and continuing education.
• 3 years of supervisory experience preferred; long term care experience preferred.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines.
• Lead weekly PDPM meetings for skilled patients and monitor PDPM tools.
• Coordinate and document MDS assessments and the resident plan of care.
