Job summary

Sweetwater Care in Tulare, CA seeks a Registered Nurse or Licensed Vocational Nurse to lead timely RAI and care management from admission to discharge in accordance with policy and federal/state guidelines. You will coordinate MDS activities, ensure accurate coding, and maintain updated MDS status across residents.

You will collaborate with the interdisciplinary team, monitor data entry into MDS programs, and support compliance with OBRA regulations and Medicaid reimbursement requirements.

Qualifications

• RN or LVN licensed in California is required.
• Experience with MDS 3.0 and RAI guidelines is preferred.
• Knowledge of OBRA regulations and PPS/Medicaid reimbursement is advantageous.
• Maintains current MDS status and accurate data entry per guidelines.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and plan MDS assessments.
• Coordinate scheduling for MDS assessments and care plan reviews per regulations.
• Support daily stand-up meetings reviewing resident care and assessment dates.
• Ensure compliance with federal and state regulations for RAI processes.
• Monitor MDS and care plan documentation in the medical record.
• Maintain accurate MDS status per guidelines and verify coding accuracy.
