Job summary

Sweetwater Care is seeking an RN with expertise in the MDS process to manage RAI and care management from admission to discharge in a Skilled Nursing Facility. You will coordinate data entry, coding, and interdisciplinary coordination to ensure compliance with state, federal, and certification guidelines.

The role emphasizes accurate MDS coding, timely updates, and collaboration with the care team to maintain quality resident records. Experience with MDS 3.0 is preferred.

Qualifications

• Must be a graduate of an approved RN program and licensed to practice in the state.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and the Minimum Data Set (MDS).
• Knowledge of the care planning process.
• Experience with MDS 3.0 preferred.
• Maintains current MDS status and timely data entry in MDS systems.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care-plan reviews.
• Decide on proceeding with or deferring MDS assessments based on clinical criteria per the RAI User’s Manual.
• Assist with coordination of daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations governing MDS processes.
• Monitor MDS and care plan documentation and maintain accurate medical records to support coding.
• Maintain current MDS status per guidelines and ensure timely data entry into MDS programs.
• Perform MDS coding using data from medical records, observations, and interviews.
• Other duties may be assigned as needed.
