Job summary

Sweetwater Care is seeking a Registered Nurse to oversee the RAI and care management processes from admission to discharge. The role ensures compliance with Federal, State, and Certification guidelines and coordinates with the Director of Nursing and VP of Clinical Reimbursement.

The ideal candidate has an RN license, at least 2 years in a Skilled Nursing Facility, and strong knowledge of MDS, OBRA, and care planning. Proficiency in MDS coding and data entry is required.

Qualifications

• RN license required in practicing state.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding of OBRA regulations and MDS.
• Experience with care planning processes and MDS coding.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS assessments are based on clinical criteria per the RAI User’s Manual.
• Assist in coordinating daily stand-up meetings to review resident care and assessment dates.
• Comply with federal and state regulations for RAI process completion and coordination.
• Maintain accurate MDS status and data entry in appropriate systems.
• Code MDS accurately using medical records, observations, and interviews.
• Other duties may be assigned as needed.
