Job summary

Fowler Care Center in Fowler, California is seeking an experienced MDS RN to manage the MDS/RAI process from admission to discharge. You will work with an interdisciplinary team to guide MDS assessments, care plan reviews, and ensure documentation accuracy in compliance with federal and state regulations.

The role requires RN/LVN licensure in the state, at least 2 years in a Skilled Nursing Facility, and strong knowledge of Medicare PPS and OBRA regulations.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and Minimum Data Set (MDS).
• Knowledge of MDS 3.0 considered a plus.

Responsibilities

• Collaborate with the Interdisciplinary Team on MDS assessments and care plan reviews.
• Guide decisions on completing MDS assessments based on clinical criteria.
• Assist with coordinating daily stand-up meetings and ARD dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation to support coding in medical records.
• Maintain current MDS status for assigned residents.
• Maintain accurate data entry of resident information in MDS systems.
• Complete accurate MDS coding from chart reviews, observations, and interviews.
