Job summary

Skilled Nursing of California is seeking an experienced RN MDS Coordinator to oversee the timely completion of the RAI process and care management from admission to discharge in line with policy and regulatory guidelines. You will coordinate data collection, MDS coding, and care planning in collaboration with the interdisciplinary team.

You will monitor MDS documentation, update resident records, and ensure accurate data entry into MDS systems, while maintaining compliance with OBRA, Medicare

Qualifications

• Must be a graduate of an approved RN program and licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, and MDS 3.0.
• Experience with MDS 3.0 and care planning processes.

Responsibilities

• Collaborate with Interdisciplinary Team to assess needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS assessments based on RAI criteria in the latest User's Manual.
• Assist with coordinating daily stand-up meetings and setting assessment reference dates.
• Comply with federal and state regulations for RAI process.
• Monitor MDS and care plan documentation; ensure records support MDS coding.
• Maintain current MDS status of assigned residents per guidelines.
• Ensure frequent data entry into MDS systems.
• Complete accurate MDS coding from records, observations, and interviews.
• Other duties may be assigned as needed.
