Job summary

Sweetwater Care is seeking an experienced nurse to manage the RAI and MDS processes from admission to discharge in a skilled nursing facility. You will coordinate interdisciplinary assessments, ensure regulatory compliance, and maintain accurate MDS coding and data entry.

The role requires RN or LVN licensure in the state, with 2+ years in SNF and strong knowledge of OBRA, Case-Mix, and Medicare/Medicaid reimbursement. Join our team to support high-quality resident care.

Qualifications

• Graduate of an approved RN or LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS process, and Medicaid reimbursement.
• Solid understanding of OBRA regulations and Minimum Data Set (MDS).
• Experience with care planning processes and MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments.
• Coordinate MDS assessments and care plan reviews per governing agency requirements.
• Assist with daily stand-up meetings and review resident care and assessment reference dates.
• Ensure compliance with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in the resident's medical record.
• Maintain current MDS status and accurate data entry in MDS programs.
• Code MDS accurately using information from medical records, observation, and staff interviews.
• Other duties may be assigned as needed.
