Job summary

Pacific Villas Post Acute in seeks an experienced Nursing Supervisor to oversee the facility's assessment processes, including MDS and the resident plan of care. You will lead PDPM-related activities, coordinate interdisciplinary MDS participation, and ensure timely, compliant documentation.

Preferred candidates hold a BSN and have at least 3 years of supervisory and long-term care experience, with active State Board of Nursing standing and ongoing CE requirements.

Qualifications

• Graduation from an Accredited School of Nursing; BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain CE/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee the resident assessment process including MDS and care planning.
• Provide PDPM and MDS training to new coordinators.
• Coordinate interdisciplinary participation in completing the MDS for new admissions.
• Ensure timely completion and accuracy of MDS assessments and regulatory submissions.
• Schedule and conduct resident care conferences and monitor care plan adequacy.
• Maintain confidentiality of resident care information and ensure data security.
