Job summary

PARKVIEW IN Denver is seeking a nurse leader to oversee the facility's resident assessments and ensure guideline-based MDS processes are followed.

You will coordinate PDPM activities, supervise MDS coordinators, train staff, and monitor QA/QM metrics to drive compliant, high-quality care.

This on-site role requires strong data entry skills, MS Office proficiency, and adherence to regulatory standards.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain required continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficiency in PCC and Microsoft Office.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines.
• Provide oversight on weekly PDPM meetings for skilled patients.
• Track and monitor facility PDPM tools and data accuracy.
• Monitor QM and QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments including PPS Medicare.
• Communicate level of care to all disciplines and coordinate MDS timelines.
• Maintain accurate schedules of MDS assessments and reference dates.
• Handle data entry for MDS submissions and verify electronic records.
• Ensure confidentiality of all resident health information.
