Job summary

Park Place is seeking an experienced MDS Coordinator/RAC to lead the MDS/RAI process in a skilled nursing facility. You will oversee timely, accurate MDS 3.0 assessments, CAAs, care plans, and documentation to support compliance and reimbursement.

Collaboration with the interdisciplinary team is essential to drive resident-centered outcomes. The ideal candidate is an RN with PDPM knowledge, CMS regulation familiarity, and expertise in MDS software and EHRs.

Qualifications

Active RN license in Washington state (RN preferred; LPN considered in some facilities).
2–3 years of clinical nursing in SNF/long-term care; prior MDS/RAI experience preferred.
In-depth knowledge of MDS 3.0, PDPM, CMS regulations, ICD-10 coding, and quality measures.
Proficiency with EHR systems and MDS software; strong organizational and communication skills.
AANAC RAC-CT certification preferred; CPR certification required.

Responsibilities

Lead MDS 3.0, CAAs, and care plan development for all residents.
Coordinate resident assessments (admission, quarterly, annual, changes, discharge).
Ensure accurate coding and transmission of MDS data to state/federal systems.
Lead interdisciplinary care plan meetings with nursing, therapy, physicians, and others.
Monitor PDPM classification and reimbursement, track quality measures and census data.
Educate staff on RAI/MDS processes, regulations, and documentation best practices.
Serve as CMS RAI compliance resource and prepare for audits.
Maintain confidentiality and ensure ethical standards in all assessments.
