Job summary

Sage Creek Post-Acute in Las Vegas, NV is seeking an experienced Nursing Supervisor to oversee the resident assessment process, including MDS and PDPM activities, and to ensure assessments are completed per guidelines.

You will lead weekly PDPM meetings, coordinate interdisciplinary teams, provide MDS training, maintain documentation accuracy, and ensure compliance with federal/state standards while safeguarding resident confidentiality.

Qualifications

• Graduation from an Accredited School of Nursing; BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long term care experience preferred.
• Proficiency in Microsoft Suite; PCC knowledge preferred.

Responsibilities

• Oversee facility's weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools usage and related data.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure the accurate and timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate interdisciplinary participation in completing the MDS within regulatory time frames.
• Maintain an accurate schedule of all MDS assessments with proper reference dates.
• Handle data entry and electronic submission of MDS assessments.
• Maintain confidentiality of all resident care information.
