Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Current RN license in good standing in the state.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee facilities' weekly PDPM meetings and monitor PDPM tool utilization.
• Track QM and QASP performance and provide data-driven recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions and ensure timely MDS completion.
