Job summary

UNGERPARK is seeking an experienced nursing leader to oversee the facility’s resident assessment program, including MDS and PDPM processes. You will coordinate interdisciplinary teams, monitor regulatory compliance, and guide data-driven care planning.

The role emphasizes supervising staff, ensuring timely MDS assessments, and training new MDS Coordinators. Bachelor of Science in Nursing preferred and active nursing licensure is required.

Qualifications

• Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years of supervisory experience preferred; Long Term Care experience preferred.

Responsibilities

• Oversees the facility's weekly PDPM meetings for skilled patients.
• Tracks and monitors use of PDPM tools and data analysis for optimal outcomes.
• Provides PDPM and MDS training to new MDS Coordinators.
• Assesses health status and level of care for new admissions and ensures timely MDS assessments.
• Coordinates interdisciplinary MDS participation and maintains regulatory documentation timelines.
• Maintains confidentiality of resident care information and ensures appropriate data entry and submissions.
