PACS, a post-acute care provider, seeks an RN supervisor to oversee PDPM and MDS processes within our Willoughby, Ohio facility. You will lead weekly PDPM meetings, monitor resource utilization, and coordinate data-driven improvements for resident care.

The role requires a BSN, active nursing license, and 3 years of supervisory experience in long-term care. Strong knowledge of regulatory standards and proficiency with Microsoft Office are essential.

Qualifikationen

• BSN degree; good standing with State Board of Nursing; continuing education/licensing requirements.
• 3 years supervisory experience preferred; long term care experience preferred.
• Knowledge of PDPM, MDS processes, and regulatory requirements.
• Proficiency with Microsoft Office and related clinical software.

Aufgaben

• Oversee weekly PDPM meetings and monitor facility utilization of PDPM tools.
• Coordinate interdisciplinary participation in completing MDS for new admissions.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of all MDS assessments and documentation.
• Maintain confidentiality of resident care information and data submissions.
