PACS seeks a Nursing Supervisor to oversee resident assessment processes, including MDS and PDPM, across facilities. You will coordinate interdisciplinary teams, monitor documentation quality, and ensure regulatory compliance.

Leadership of the MDS team and ongoing staff training are key responsibilities. The role emphasizes accurate data entry, timely MDS submissions, and ensuring care plans are comprehensive and aligned with state and federal standards.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsabilités

• Oversight of weekly PDPM meetings for skilled patients.
• Track and analyze PDPM tools usage and outcomes.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate interdisciplinary participation in completing MDS for each new admission.
• Maintain documentation to meet federal and state standards.
• Manage level of care changes and notify departments accordingly.
