Concordia-Manor is seeking a Nursing supervisor to oversee the facility's resident assessment processes, including MDS, PDPM, and care planning. The role focuses on ensuring guidelines are followed and assessments are completed accurately and on time.

Responsibilities include leading PDPM meetings, training new MDS Coordinators, and coordinating interdisciplinary teams while maintaining documentation integrity and regulatory compliance.

Qualificações

• BSN preferred, in good standing with the State Board of Nursing.
• Minimum 3 years of supervisory experience preferred.
• Long Term Care experience preferred.
• Proficient with Microsoft Office; PCC knowledge a plus.

Responsabilidades

• Oversee weekly PDPM meetings for skilled patients.
• Track PDPM tools usage and report performance.
• Provide PDPM and MDS training to new coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of MDS assessments and documentation.
• Coordinate interdisciplinary MDS activities within regulatory timeframes.
• Maintain confidentiality of resident information per regulations.
