Job summary

Unity Health and Rehabilitation Center in Miami, FL is seeking an experienced MDS Coordinator to lead patient assessment processes and care planning in our long-term care setting. You will collaborate with interdisciplinary teams to develop individualized treatment plans, analyze data, and ensure accurate records while maintaining compliance with state regulations.

A strong background in MDS and EMR systems is required.

Qualifications

Current Florida RN license is required.
Minimum 1 year experience in a long-term care setting as an MDS Coordinator.
Strong analytical and problem-solving abilities are essential.
Excellent communication and interpersonal skills are necessary.
Ability to work effectively in a fast-paced environment.
Familiarity with MDS software and electronic medical records is required.

Responsibilities

Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for residents.
Collect and analyze data to identify trends and opportunities to improve patient care.
Develop and maintain accurate and timely records of patient care and treatment.
Assist with implementing individualized treatment plans and monitor patient progress.
Participate in quality improvement initiatives to enhance patient outcomes.
Maintain effective communication with patients, families, and the healthcare team.
