Job summary

Manchester Center for Rehabilitation and Healing is seeking an MDS Coordinator to join our skilled nursing facility team. The successful candidate will oversee MDS, CAAs and care plans in line with federal and state regulations, acting as the in-house case manager to communicate with residents, families and other care professionals.

The role requires licensure, strong assessment skills and experience in geriatric care.

Qualifications

• Licensed nurse with active license in state of practice.
• 2+ years clinical experience in a health care setting.
• 1+ year of experience in long-term care setting.
• Prior experience as an MDS coordinator is preferred or accepted.
• Training program available for candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate completion of the Minimum Data Set (MDS) and related assessments and care plans.
• Acts as in-house Case Manager, communicating with residents, families, other health professionals and payers.
• Collaborates with Administrator and Director of Nursing to identify regulatory risks and process improvements.
• Selects correct ARD and captures RUG score reflecting care and services provided.
• Monitors MDS requirements for various payers including Medicare, managed care and Medicaid.
• Ensures timely electronic submission of all MDS to the state database and reviews validation reports.
• Leads Care Management process with resident, IDT and family to resolve discharge barriers.
• Educates IDT members on RAI/MDS processes and trains staff accordingly.
• Analyzes QM/QI data with the DON to identify trends.
• Responsible for timely Utilization Review and Triple Check.
• Participates in committees as required by supervisor.
