Silver Tree Nursing & Rehabilitation is seeking an experienced MDS Coordinator to lead resident assessments and ensure accurate documentation. You will collaborate with nursing and other departments to ensure timely MDS submissions and compliant care planning.

The role requires strong organizational skills, attention to detail, and a focus on resident-centered care, with ongoing professional development to stay up-to-date on regulations.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance with federal/state MDS guidelines.
• Monitor MDS and care plan documentation for accuracy and completeness.
• Utilize MDS software to ensure correct data entry and reporting.
• Lead interdisciplinary meetings for care planning and communication.
• Update and revise care plans based on assessments and resident status.
• Generate and distribute Quality Indicators reports to leadership.
• Submit MDS data per reporting standards.
• Stay current with industry standards through professional development.
• Oversee ancillary data reporting to corporate billing.
