MDS Coordinator (Long-term Care)
Job Summary:
Unity Health and Rehabilitation Center, Setting standards in Rehabilitation and Skilled Nursing Care, is seeking a dedicated, compassionate and experienced MDS Coordinator to join our team. As a vital member of our care delivery team, you will be responsible for ensuring the highest quality of care for our long-term care patients.
About the Role:
As an MDS Coordinator, you will be responsible for:
- Collecting and analyzing data to develop a comprehensive care plan for each patient
- Coordinating with care providers, therapists, and other healthcare professionals to ensure seamless care delivery
- Assisting with the development and implementation of individualized treatment plans
- Monitoring patient progress and making adjustments to care plans as necessary
- Maintaining accurate and timely records of patient care and treatment
Responsibilities:
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Gather and analyze data to identify trends and areas for improvement in patient care
- Develop and maintain accurate and timely records of patient care and treatment
- Assist with the implementation of individualized treatment plans and monitor patient progressParticipate in quality improvement initiatives to enhance patient care and outcomes
- Maintain effective communication with patients, families, and healthcare team members
Requirements:
- Current certification as a Licensed Practitioner (LPT) or Registered Nurse (RN) in the state of Florida
- Minimum of 1 year experience in a long-term care setting, as an MDS Coordinator
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Familiarity with MDS software and electronic medical records
