Job summary

Unity Health and Rehabilitation Center is seeking an experienced MDS Coordinator to lead data-driven care planning for long-term care patients. You will collect and analyze data, coordinate with care providers, and support the development and implementation of individualized treatment plans.

Requirements include a Florida RN license (or LPT), at least 1 year in a long-term care setting, and strong analytical and communication skills plus familiarity with MDS software and EMR systems.

Qualifications

• Current certification as a Licensed Practitioner (LPT) or Registered Nurse (RN) in Florida.
• Minimum of 1 year experience in a long-term care setting as an MDS Coordinator.
• Strong analytical and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
• Gather and analyze data to identify trends and areas for improvement in patient care.
• Develop and maintain accurate and timely records of patient care and treatment.

Job Summary:

As a vital member of our care delivery team, you will be responsible for ensuring the highest quality of care for our long-term care patients.

About the Role:

As an MDS Coordinator, you will be responsible for:

• Collecting and analyzing data to develop a comprehensive care plan for each patient
• Coordinating with care providers, therapists, and other healthcare professionals to ensure seamless care delivery
• Assisting with the development and implementation of individualized treatment plans
• Monitoring patient progress and making adjustments to care plans as necessary
• Maintaining accurate and timely records of patient care and treatment
