Creative Solutions In HealthCare is seeking an MDS Coordinator to ensure accurate assessments and compliant care planning for residents. You will collaborate with nursing and interdisciplinary teams to manage admissions, annual and change-of-condition assessments, and data reporting.

The role requires strong organizational skills, reliability, and a commitment to resident-centered care in a geriatric population. This is a full-time on-site position with opportunities for professional development.

Qualifications

• Must hold a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.
• Ability to work independently and in a team.
• Ability to manage multiple priorities with accuracy and efficiency.

Responsibilities

• Oversee admission and annual MDS assessments and communication with care teams.
• Ensure compliance with federal and state MDS guidelines.
• Review MDS and care plans for accuracy and completeness.
• Maintain proficiency in MDS software for data entry and reporting.
• Lead interdisciplinary meetings for care planning and coordination.
• Update care plans based on assessments and resident status.
• Generate quality indicator reports for leadership and compliance.
• Submit MDS data per reporting standards.
• Pursue professional development to stay current.
• Report ancillary data to corporate billing as required.
