Job summary

Monarch Healthcare Management in Minnesota is seeking an MDS Coordinator (RN or LPN) to oversee the completion, editing, locking and transmission of all MDS assessments, ensuring timely, accurate documentation for reimbursement and regulatory compliance.

You will coordinate care plan completion with the Interdisciplinary Team, provide clinical support to nursing staff, and leverage MDS/RAI experience to maximize quality of care across Monarch's network.

Qualifications

• Must be a Registered nurse (RN), or Licensed Practical Nurse (LPN) in Minnesota.
• CPR certification and emergency care training required.
• Ability to communicate verbally and in writing during RAI process.
• Must maintain regulatory compliance and accuracy in documentation.
• Healthcare experience in skilled nursing or assisted living preferred.
• Experience with MDS and RAI processes preferred.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely, accurate MDS completion for reimbursement and compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to enhance care quality.
• Leverage MDS/RAI experience to maximize care quality and reimbursement.
