Job summary

Monarch Healthcare Management is seeking an LPN to coordinate MDS processes, edit and transmit assessments, and support regulatory compliance. The role focuses on timely, accurate MDS completion, maximizing case mix, and coordinating care plans with the Interdisciplinary Team.

You will support nursing staff and participate in PIDM activities to ensure quality care and reimbursement alignment across Monarch's facilities in Minnesota.

Qualifications

• Must be an LPN in Minnesota with CPR certification.
• Ability to communicate verbally and in writing when directing the RAI process.
• Capable of multitasking and adapting to changing demands.

Responsibilities

• Complete admission chart reviews and input ICD-10 codes for billing.
• Oversee timely MDS completion and coordinate IDT care plans.
• Support Medicare/recertification documentation and eligibility monitoring.
• Assist with staff training to improve MDScoding accuracy.
• Participate in regulatory meetings and QAPI-related activities.
