Ideal candidates bring MN RN licensure, CPR certification, and experience with MDS/RAI in healthcare settings. The role emphasizes timely, compliant processes and ongoing collaboration across Monarch's facilities.

Qualifications

• Must be a Registered nurse (RN) in Minnesota.
• CPR certification required or pursuing.
• Ability to communicate verbally and in writing when directing the RAI process and assessing residents.
• Experience in healthcare, skilled nursing facility or assisted living.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely completion of the MDS with focus on accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to improve assessments and quality of care.
