Monarch Healthcare Management is seeking a skilled MDS Coordinator in Minnesota to oversee MDS assessments from editing to transmission, ensuring accuracy and regulatory compliance. You will support care plans with the Interdisciplinary Team and provide clinical support to nursing staff.

Qualifications

• Must be a Registered nurse (RN) in Minnesota.
• CPR certification or eligible for certification.
• Excellent verbal and written communication for RAI direction.
• Experience in healthcare, skilled nursing, or assisted living.
• Experience with MDS and RAI processes.

Responsibilities

• Oversee completion, editing, locking and transmission of MDS assessments.
• Ensure timely, accurate MDS completion focused on reimbursement and compliance.
• Coordinate care plan completion with IDT and support nursing staff.
