Job summary

Monarch Healthcare Management is seeking a Minimum Data Set (MDS) Coordinator to oversee the MDS assessment lifecycle, ensuring accuracy, timely completion, and regulatory compliance across our Minnesota facilities. You will collaborate with the IDT to finalize care plans and provide clinical support to nursing staff to maximize quality of care and compliance.

A Minnesota RN or LPN license with CPR certification is required.

Qualifications

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota.
• CPR certification and emergency care for the health care provider is required or pursued.
• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee the completion, editing, locking and transmission process of all MDS assessments.
• Focus on timely completion of the MDS with accuracy, case mix maximization, and regulatory compliance.
• Coordinate care plan completion with the IDT and render clinical support to nursing staff to enhance accurate assessments and quality of care.
