Job summary

Monarch Healthcare Management seeks an LPN MDS Coordinator in Minnesota to ensure timely, accurate MDS completion with a focus on PDPM and regulatory compliance.

You will collaborate with the Interdisciplinary Team to complete care plans and provide clinical support to nursing staff, helping optimize reimbursement and quality of care across Monarch's facilities. This role emphasizes attention to detail, communication, and adherence to care standards.

Qualifications

• Must be a Licensed Practical Nurse (LPN) in Minnesota.
• CPR certification and emergency care for health care provider required.
• Must communicate clearly in writing and verbally when directing the RAI process.
• Must perform essential functions with or without reasonable accommodation.
• Criminal background check required per Minnesota DHS guidelines.

Responsibilities

• Edit, lock, and transmit all MDS assessments timely.
• Support accurate MDS completion, case mix maximization, and regulatory compliance.
• Coordinate care plan completion with Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff for accurate assessments and care plans.
• Oversee PDPM review processes and Medicare documentation to support reimbursement.
• Conduct IDT Case Management meetings and monitor Quality Measures.
