Monarch Healthcare Management in Minnesota is seeking an MDS Coordinator to oversee the completion, editing, locking and transmission of all MDS assessments at our Eveleth and Two Harbors locations. The role focuses on timely completion with accuracy and regulatory compliance.

You will coordinate care plan completion with the IDT, render clinical support to nursing staff, and help maximize case mix while ensuring high quality of care for residents.

Qualifications

• Must be able to communicate verbally and in writing when directing the RAI process and when performing and assessing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee the completion, editing, locking and transmission process of all MDS assessments.
• Ensure timely completion of the MDS with a focus on accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT) and render clinical support to nursing staff to enhance assessments and quality of care.

Education

RN or LPN license in Minnesota

CPR certification or health care provider certification
