Job summary

Monarch Healthcare Management in Minnesota is seeking a dedicated MDS Coordinator to oversee the completion, editing, locking and transmission of all MDS assessments, ensuring timely, accurate care plans and regulatory compliance.

You will coordinate care plans with the Interdisciplinary Team, render clinical support to nursing staff, and help maximize case mix while maintaining high-quality resident care across Monarch's facilities.

Qualifications

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota.
• Certification in CPR and emergency care for the health care provider is preferred or in progress.
• Ability to communicate verbally and in writing when directing the RAI process and when performing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee the completion, editing, locking and transmission of all MDS assessments.
• Support timely completion of the MDS with focus on accuracy, case mix maximization and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT) and render clinical support to nursing staff to enhance assessments and quality of care.
