Job summary

Monarch Healthcare Management is seeking a Minimum Data Set (MDS) Coordinator to oversee all MDS assessment processes, ensuring accuracy and timely submission while complying with regulations. You will support nursing staff and coordinate care plans with the Interdisciplinary Team to maximize quality of care.

Requirements include MN RN or LPN licensure and CPR certification, with strong communication skills for directing the RAI process and resident care assessments.

Qualifications

• Must hold RN or LPN license in Minnesota.
• CPR/EC provider certification required or pursued.
• Ability to communicate clearly both verbally and in writing while directing RAI processes.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely MDS completion with accuracy and regulatory compliance.
• Coordinate care plan completion with the IDT and provide clinical support to nursing staff.
