Job summary

SNF Staffing Solutions is seeking a Registered Nurse to serve as MDS Coordinator, overseeing timely MDS completion and care management from admission to discharge in line with policy and regulations. The role reports to nursing leadership and involves coordinating information systems and education for the clinical department.

The position requires RN licensure in the state, with preferred experience in MDS 3.0 and Medicare/Medicaid reimbursement processes.

Qualifications

• RN license in the applicable state is required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and MDS requirements.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with the Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS per criteria in RAI manual.
• Coordinate daily stand-up on resident care and assessment reference dates.
• Ensure compliance with federal/state regulations for MDS processes.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per state and federal guidelines.
• Enter resident data into computerized MDS programs accurately.
• Code MDS data from chart review, observations, interviews.
• Attend interdisciplinary, QA, and other meetings to update records.
• Assist DON with significant change identification and communication.
• Prepare scheduling and notices for care conferences; communicate outcomes.
• Keep knowledge base updated on data entry and computer tech.
• Submit electronic documentation to state database per policy.
• Correct and finalize MDS and submit to state/federal agencies.
• Instruct staff in RAI Process, PPS Medicare/Medicaid and clinical systems.
• Maintain confidentiality.
• Other duties as assigned.
