Care Initiatives is seeking an MDS Coordinator (RN) to coordinate accurate MDS assessments and collaborate with interdisciplinary teams to develop individualized care plans. The role emphasizes compliance with CMS and state regulations and ensuring high-quality documentation for proper reimbursement.

The RN will monitor clinical records, support care conferences, and act as a resource for nursing staff on MDS processes, infection control, and survey readiness.

Formación

• Active Iowa RN license in good standing.
• Current CPR certification.
• Knowledge of federal and state long-term care regulations.
• Strong communication and teamwork skills.
• Desire to learn and grow within a quality-focused clinical team.

Responsabilidades

• Complete and coordinate accurate, timely MDS assessments.
• Collaborate with interdisciplinary teams to update individualized care plans.
• Ensure CMS, state, and federal long-term care regulations are met.
• Monitor and validate clinical documentation for reimbursement and quality measures.
• Analyze resident data to support care quality and clinical decision-making.
• Serve as a resource on MDS processes and documentation standards.
• Participate in care conferences and support communication with residents and families.
• Support audit readiness and maintain survey-ready documentation at all times.
• Enforce policies and guide the team, fostering personal and team growth.
• Communicate effectively with residents and families regarding care needs.
• Document care accurately to meet clinical, regulatory, and facility standards.
• Follow infection control, safety, and compliance protocols.
• Participate in an on-call rotation with other leadership team members.
