Zusammenfassung

SNF Staffing Solutions seeks an experienced MDS Coordinator to oversee timely completion of the RAI and care management processes from admission to discharge. You will work under the DON and VP of Clinical Reimbursement, coordinating information systems and staff education in the clinical department.

You will collaborate with the interdisciplinary team to schedule MDS assessments, ensure compliance with regulations, and maintain accurate documentation across state and federal requirements.

Qualifikationen

• Graduate of RN program and licensed in practice state.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Thorough knowledge of MDS 3.0, Medicare PPS, and Medicaid reimbursement.
• Understanding OBRA, Quality Indicator process, and data accuracy.

Aufgaben

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions on additional MDS assessments follow clinical criteria per the RAI manual.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Comply with federal/state regulations for RAI process completion.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status and data entry into computerized MDS programs.
• Code final MDS from medical records, observations, and interviews.
• Attend interdisciplinary meetings and communicate changes; update records.
• Identify significant changes with DON/designee and reflect them in MDS and care plans.
• Prepare scheduling and notice of care planning conferences; communicate outcomes.
• Continually update knowledge of data entry and computer technology.
• Submit required documentation electronically to state databases per policy.
• Assign and instruct staff on RAI Process, PPS Medicare/Medicaid, and clinical systems.
• Maintain confidentiality; adapt to other duties as needed.
