Job summary

Majestic Care is seeking an MDS Coordinator (RN) to supervise MDS and care plans for residents, ensuring timely and compliant documentation. The role requires active RN licensure, familiarity with MDS 3.0, and proficiency in data entry and word processing.

Responsibilities include coordinating MDS timelines, PPS and state assessments, and leading care plan meetings with the care team, while upholding resident rights and privacy. Strong organizational skills and compliance mindset are essential.

Qualifications

• Must hold current RN or LPN nursing license in the state of employment; license must be active, valid, and in good standing.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing required.
• Knowledge of nursing practices, procedures, laws, regulations for skilled nursing facilities.
• Ability to plan, organize, develop, implement and interpret programs, goals, policies and procedures.
• Must hold an active CPR license or obtain within 30 days of employment.

Responsibilities

• Coordinate the MDS and care plans for residents per facility policies.
• Inform care team when a care plan is due and ensure completion of MDS portions.
• Develop and revise resident care plans quarterly or with condition changes.
• Handle PPS and state MDS assessments; ensure MDS, CATs, CAAs are completed.
• Coordinate care plan meetings and encourage team participation.
• Ensure timely submission of MDS to state/federal agencies per guidelines.
• Maintain resident rights, confidentiality, and privacy in the MDS process.
• Maintain communication across departments to keep MDS and care plans current.
• Monitor resident care during MDS and report concerns to the Executive Director and Director of Nursing.
• Assist with pre-admission screening to estimate potential RUGs.
