OPCO Skilled Management seeks an experienced MDS Coordinator to ensure timely, accurate MDS completion from admission to discharge under policy and federal/state guidelines. You will work with the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and staff education.

Responsibilities include interdisciplinary care planning, regulatory compliance, data entry into MDS programs, and communication of outcomes to staff and families.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process, OBRA regulations and Minimum Data Set (MDS).
• Knowledge of the care planning process.
• Experience with MDS 3.0 is preferred.

Responsabilités

• Collaborate with the interdisciplinary team to assess residents and schedule MDS assessments and care plan reviews as required by agencies.
• Decide when additional MDS assessments are needed based on clinical criteria and MDS User's Manual.
• Coordinate daily stand-up meetings focusing on resident care and assessment reference dates.
• Ensure compliance with federal and state regulations governing MDS processes.
• Monitor and document MDS and care plans in the medical records to support coding.
• Maintain current MDS status for residents per guidelines.
• Enter resident data into MDS software accurately and timely.
• Code MDS data from medical records, observations, and interviews with staff, residents, and families.
• Attend interdisciplinary team meetings to relay changes and update records.
• Assist with identifying significant changes and communicate physician orders for accurate MDS reflection.
• Prepare scheduling and notices for care conferences and communicate outcomes to staff and residents.
