Job summary

SNF Staffing Solutions is seeking a Registered Nurse with MDS coordination expertise to manage the RAI and care management processes from admission to discharge in compliance with federal and state guidelines.

The role requires collaboration with an interdisciplinary team, familiarity with MDS 3.0, OBRA, and PPS, and a focus on accurate data entry within state databases and care plans.

Qualifications

• Registered Nurse license in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
• Thorough understanding of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Knowledge of OBRA regulations and Minimum Data Set (MDS 3.0) is advantageous.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by agencies.
• Decide on whether to complete additional MDS assessments based on clinical criteria and MDS guidelines.
• Coordinate daily stand-up meetings to review resident care and set assessment reference dates.
• Ensure compliance with federal and state regulations for MDS processes.
