Schnepp Senior Care & Rehab is seeking an experienced MDS Coordinator RN to lead the RAI process and ensure accurate, timely completion of assessments. You will enter data into the electronic records system and monitor compliance with state and federal regulations.

The role emphasizes mentorship, documentation accuracy, and collaboration with the interdisciplinary team to uphold EPIC care values and provide high-quality care.

Qualifications

• Graduate from an accredited school of nursing with 5+ years of related MDS experience.
• Current RN license in the state and CPR-BLS certification.
• Strong organizational, time-management and attention to detail.

Responsibilities

• Coordinate the RAI process and timely completion of the MDS CAA’s interdisciplinary care plan.
• Complete the Resident Assessment Instrument accurately and on time.
