Apploi is seeking a MDS Coordinator to oversee accurate and timely clinical assessments within a facility care team. The role supports improvement goals through resident assessments, MDS completion, and care planning in compliance with state and federal regulations.

The incumbent will lead interdisciplinary meetings, manage the RAI process, and contribute to PDPM discussions, with a focus on quality resident care and data integrity.

Qualifikationen

• RN license in Connecticut is required.
• Minimum of three years in geriatric nursing or rehabilitation preferred.
• CPR certification and ability to read, write, and understand English.

Aufgaben

• Maintains care conferences calendar and conducts interdisciplinary meetings as needed.
• Performs comprehensive resident assessments via medical record review and interviews.
• Completes MDS with RAPS and resident summaries, and develops care plans.
• Ensures timely completion of the RAI process and transmission per regulations.
• Participates in CMI, billing meetings, and communicates PDPM recommendations.
• Attends meetings as assigned by the DNS/ADNS and maintains resident data confidentiality.
