Job summary

Pine Grove Manor Skilled Nursing is seeking an experienced MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring all documentation meets Federal, State, and Certification guidelines.

The role manages the RAI process, validates MDS, CAAs, and the Interdisciplinary Care Plan, coordinates audits, and tracks submissions in PointClickCare to support accurate payment data and timely regulatory compliance.

Qualifications

• Nursing degree and state RN/LVN license.
• 2+ years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA and MDS processes.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Participates in the regulatory or certification survey process and the correction of deficiencies.
• Reports trends from completed audits to the Quality Assurance Committee.
• Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the PointClickCare system.
• Follows up with staff to assure compliance to documentation standards.
• Completes patient assessments, data collection, and interviews staff to assure good practice per MDS User’s Manual.
• Facilitates accurate determination of the Assessment Reference Date reflecting patient needs and resources for Medicare/Medicaid and insurance payments.
• Ensures timely submission of the MDSs to the State with proper follow-up on validation errors and maintains records.
