Job summary

OPCO Skilled Management seeks an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. The role coordinates the RAI process to assure timely, complete MDS, CAAs, and the Interdisciplinary Care Plan, tracking forms in PointClickCare and reporting trends to the Quality Assurance Committee.

A registered nurse or licensed practical nurse licensed in the state is required, with at least 2 years in

Qualifications

• Graduate of an approved RN/LPN program and licensed in the state.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations, and care planning.

Responsibilities

• Assist in assuring adherence to Federal and State regulations and certification.
• Participate in the regulatory or certification survey process and correct deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Assure completion of the RAI Process from the MDS through the interdisciplinary care plan.
• Initiate and monitor RAI process tracking and Medicaid tracking forms in PointClickCare.
• Follow up with staff to ensure documentation standards are met.
• Complete patient assessments and data collection; interview staff as needed.
• Facilitate accurate Assessment Reference Date determination reflecting care needs.
• Ensure timely submission of MDSs to the State with validation follow-up.
