Job summary

OPCO Skilled Management is seeking a qualified MDS Coordinator to support the Director of Nursing in a skilled nursing facility in Missouri. You will ensure documentation aligns with federal, state, and certification guidelines and coordinate the RAI process from MDS through the interdisciplinary care plan.

You will monitor audits, manage MDS submissions in PointClickCare, and facilitate timely completion of assessments, CAAs, and care plans.

Qualifications

• RN or LVN license in state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Understanding of OBRA regulations and MDS process.
• Knowledge of care planning process.

Responsibilities

• Assist the Director of Nursing with ensuring documentation meets guidelines.
• Coordinate the RAI process, MDS, CAAs, and interdisciplinary care plan.
• Participate in regulatory surveys and corrective actions as needed.
• Monitor MDS submissions and follow up on validation errors.
• Report trends from audits to the Quality Assurance Committee.
• Ensure timely submission of MDS to State and related entities.
