Job summary

OPCO Skilled Management's facility seeks an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation aligns with Federal, State, and Certification guidelines. You will coordinate the RAI process, ensure timely MDS submissions, and maintain validation records while supporting the interdisciplinary care planning process.

Requires RN/LVN license in the state of practice and at least 2 years of skilled nursing facility nursing experience; knowledge

Qualifications

• Nursing experience in skilled nursing facility preferred.
• Knowledge of OBRA regulations and Minimum Data Set (MDS).
• Understanding of the Federal Medicare PPS process and Medicaid reimbursement.
• Knowledge of the care planning process and quality indicators.

Responsibilities

• Assist with adherence to Federal and State regulations and certification.
• Participate in regulatory surveys and correction of deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Ensure completion of the RAI process from MDS through the interdisciplinary care plan.
• Initiate and monitor RAI process tracking, discharge/reentry and Medicaid tracking forms.
• Follow up with staff to assure compliance with documentation standards.
• Complete patient assessments, data collection, and staff interviews.
• Determine the Assessment Reference Date reflecting patient care needs and payment resources.
• Ensure timely submission of MDSs to the State and maintain validation records.
