Job summary

Opco-Skilled-Management- is seeking a skilled MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation complies with Federal, State, and Certification guidelines. This role coordinates the RAI process from MDS through interdisciplinary care planning.

The incumbent will monitor and track RAI and Medicaid forms in the PointClickCare system, ensure timely MDS submissions, and assist in survey preparation and deficiencies corrections.

Qualifications

• Graduate of an approved Registered Nurse / Licensed Vocational Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations, and MDS care planning.

Responsibilities

• Assists in adherence to Federal and State regulations and certification.
• Participates in regulatory survey process and correction of deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Ensures timely submission of the MDS to the State and follows up on validation errors.
• Manages RAI process tracking and Medicaid tracking forms in PointClickCare.
• Follows up with staff to ensure documentation standards.
• Completes patient assessments, data collection, and staff interviews as needed.
• Determines Accurate Assessment Reference Date reflecting care needs and resources.
