Job summary

Pine Grove Manor in Missouri is seeking an experienced MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator. The role focuses on ensuring accurate documentation and adherence to regulatory guidelines across the MDS, CAAs, and care plans.

You will coordinate the RAI process, monitor forms in PointClickCare, and work with staff to maintain high standards of care and accurate Medicare/Medicaid billing records.

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process.
• Knowledge of OBRA regulations and the Minimum Data Set.
• Knowledge of the care planning process.

Responsibilities

• Assists the center in ensuring documentation meets Federal, State, and Certification guidelines.
• Coordinates the RAI process to ensure timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking in PointClickCare.
• Reports trends from completed audits to the Quality Assurance Committee.
• Ensures timely submission of the MDSs to the State with follow-up on validation errors and maintains records.
• Follows up with staff to assure compliance with documentation standards.
• Completes patient assessments and data collection as instructed in the MDS User’s Manual.
