Job summary

The Thorny Nursing Center in Fort Worth, TX is seeking an experienced MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator in ensuring documentation meets Federal, State and Certification guidelines.

You will coordinate the RAI process, ensure timely MDS, CAAs, and the Interdisciplinary Care Plan, track validation errors, and guide staff to maintain high standards of documentation and compliant patient care.

Qualifications

• Licensed RN or LVN in the state of practice.
• 2+ years nursing experience in Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and MDS process.

Responsibilities

• Assist Director of Nursing and RN Assessment Coordinator to ensure documentation meets guidelines.
• Coordinate RAI process for timely and complete MDS, CAAs, and interdisciplinary care plan.
• Support regulatory survey processes and address deficiencies.
• Report trends from audits to Quality Assurance Committee.
• Ensure timely submission of MDSs to the State and maintain validation records.
