Arlington Heights Health in Fort Worth, TX seeks an MDS Coordinator to ensure accurate assessments and compliant care planning for residents. You will manage admission, annual, quarterly, Medicare and change-of-condition assessments, communicating with nursing and other departments to support high-quality resident care.

The role requires a current Nursing License, geriatric nursing experience, and strong assessment, documentation and coordination skills.

Qualifications

• Current Nursing License in good standing is required.
• Geriatric nursing experience and resident-centered care commitment.
• Excellent assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance requirements.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change-of-condition MDS assessments with timely communication to nursing and other departments.
• Ensure compliance with Federal and State guidelines for MDS.
• Review MDS and care plan files for accuracy and organization.
• Maintain expertise in MDS software for correct data entry and reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings for team communication.
• Update and revise care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Ensure timely transmission of MDS data per reporting standards.
• Attend professional development sessions to stay updated on industry standards.
• Collect and report ancillary data to corporate billing for accurate processing.
